If you experience any difficulties filling out this form or if you are a current registrant in one of our programs and wish to continue into the next curriculum, contact your ECHO administrator or contact us at echo.ontario@uhn.ca for assistance.
We encourage you to participate by:
- Attending as many ECHO sessions as you are able to.
- Case Presentations are an integral part of ECHO sessions. There is no limit to the number of cases you can present. You can present your own patient case or present on behalf of your team. We encourage all participants to present at least 1 case.
- Completing 2 questionnaires (The first questionnaire will be sent to you before you start your first session and the second after attending 8-10 sessions.) This helps us improve your learning experience.
