ECHO Chronic Pain & Opioid Stewardship In Person Skills Workshop

ECHO Chronic Pain & Opioid Stewardship & ECHO Rheumatology invite ECHO participants to an in-person hands-on “Bootcamp” workshop.
The workshop is geared to helping ECHO participants enhance and practice skills discussed in the weekly ECHO sessions.  Workshop sessions are developed based on participant feedback. Bootcamp is an accredited learning activity.

— Friday 29th November, 2019 • 12:00pm – 4:00pm 
— Saturday 30th November, 2019 • 8:15am – 4:00pm
Location: Toronto Rehabilitation Institute, 550 University Ave, Toronto, Ontario M5G 2A2.

ATTENDANCE
Open to ECHO participants*.
    *An ECHO participant is a registered ECHO participant, that has have attended a minimum of 3 sessions ECHO Ontario UHN Chronic Pain OR Rheumatology between Jan 1 2018 – Nov 28, 2019. (This includes evening sessions).

COST: There is no registration fee.

WHAT TO WEAR:  Please wear loose comfortable clothing & shoes, as this is a hands-on workshop.

FOCUS GROUP
We would like to collect some additional feedback about our sessions.  Would you be interested in participating in a 1-hour optional focus group?
— Friday afternoon (4:15-5:15 pm) for ECHO Chronic Pain participants 
— Saturday afternoon (4:15-5:15pm) for ECHO Rheumatology participants.
You will be compensated for your time.  This is open to ECHO participants only.

REIMBURSEMENT GUIDELINES

To be eligible for reimbursement:
You must have attended 3 ECHO Ontario UHN Chronic Pain/Opioid Stewardship or Rheumatology sessions between Jan 1 2018 – Nov 28 2019. (This includes evening sessions). NO EXCEPTIONS.
Accommodations
  • Accommodations at the Chelsea Hotel ONLY for the nights required. (Additional nights will not be covered).
  • Room must be a standard room
  • No additional charges will be covered.
  • You must live over 60km from the event to be eligible for accommodation reimbursement.

 Note: If you live closer than 60km, you can stay at the Chelsea Hotel at the special rate, but you will not be reimbursed.
If there is no availability left at the Chelsea, we will pay up to $175.00 at another hotel for the nights required. We do not reimburse Airbnb rooms.

  • Flight/Train: Must be economy class
  • Driving: Mileage will be reimbursed at a rate of 0.40c/km
  • Parking will be reimbursed
Meals
  • Meals will be provided during the event.
  • Meals/Food will not be reimbursed
Notes All reimbursement guidelines align with UHN reimbursement policies
Process
  • Reimbursement forms will be distributed at the workshop.
  • You must submit your original receipts with the reimbursement form.  Itemized receipts for the hotel is required
  • Reimbursement forms without receipts will not be accepted.
  • Please allow for 6-8 weeks from submission for reimbursement
